Job Overview
White Grape Community Services is seeking a qualified and dependable Qualified Mental Health Professional (QMHP) to join our outpatient behavioral health team.
The QMHP is responsible for completing mental health assessments, diagnostic evaluations, person-centered treatment/service planning, clinical documentation, ongoing clinical review, and other QMHP-level services in accordance with OAR 309-019, applicable Oregon requirements, organizational policies, and the individual’s professional scope and qualifications.
The QMHP will work collaboratively with QMHA staff, peer support specialists, care coordination, and other members of the treatment team to ensure members receive appropriate, timely, and person-centered behavioral health services.
Primary Responsibilities
Assessments & Diagnosis
- Conduct comprehensive mental health assessments within required timeframes.
- Gather and document relevant behavioral health, medical, social, substance-use, trauma, and treatment history as clinically appropriate.
- Complete and document mental status examinations.
- Assess symptoms, functioning, strengths, barriers, risks, and behavioral health needs.
- Conduct suicide, self-harm, violence, and other appropriate risk assessments.
- Establish and document DSM diagnoses within the QMHP’s qualifications and scope.
- Identify potential co-occurring mental health and substance-use needs.
- Determine whether members are appropriate for the program or require referral to another level of care.
Treatment & Service Planning
- Develop individualized, person-centered treatment/service plans based on assessment findings.
- Establish measurable goals, objectives, interventions, and expected outcomes.
- Ensure recommended services address identified behavioral health needs.
- Review and update treatment/service plans within required timeframes and when clinically indicated.
- Complete required QMHP signatures, recommendations, and approvals within professional scope.
- Ensure members participate in treatment/service planning as required.
Ongoing Clinical Services
- Provide individual and/or group behavioral health interventions when assigned and within professional scope.
- Monitor member symptoms, functioning, progress, and response to services.
- Reassess clinical needs when significant changes occur.
- Develop or update safety and crisis plans when clinically indicated.
- Make appropriate referrals when members require services outside the program’s scope.
- Coordinate care with other providers and members of the treatment team when appropriate and authorized.
Clinical Documentation
- Complete all assigned clinical documentation accurately and within required timeframes.
- Document assessments, diagnoses, treatment/service plans, interventions, progress, and clinical recommendations.
- Ensure documentation supports the clinical rationale and medical necessity for services.
- Maintain accurate member records in the agency’s electronic health record (EHR).
- Correct incomplete or deficient clinical documentation when identified.
- Maintain HIPAA compliance and member confidentiality.
Clinical Review & Team Coordination
- Review assigned member cases and identify concerns requiring clinical follow-up.
- Communicate clinical recommendations to appropriate members of the treatment team.
- Collaborate with QMHA staff, peer support specialists, care coordinators, and other program staff.
- Participate in case consultations and clinical meetings as required.
- Provide appropriate QMHP-level clinical direction regarding assigned member treatment and service needs.
- Immediately escalate significant safety concerns, changes in condition, or other urgent clinical issues according to agency procedures.
Compliance & Professional Responsibilities
- Provide services consistent with OAR 309-019 and applicable Oregon behavioral health requirements.
- Practice only within the individual’s education, training, credentials, qualifications, and professional scope.
- Maintain professional boundaries with members.
- Follow mandatory reporting requirements.
- Follow White Grape Community Services policies and procedures.
- Maintain required credentials and qualifications throughout the engagement.
- Participate in required training, quality review, and compliance activities.
- Complete assigned clinical responsibilities and documentation by applicable deadlines.
Minimum Qualifications
Candidates must meet the applicable QMHP qualification requirements under OAR 309-019.
Candidates should possess:
- Appropriate qualifying graduate-level education and/or credentials necessary to meet Oregon QMHP requirements.
- Knowledge of behavioral health assessment and DSM diagnostic criteria.
- Knowledge of person-centered treatment and service planning.
- Ability to conduct and document clinical and risk assessments.
- Knowledge of behavioral health documentation standards.
- Experience working with individuals experiencing mental health and/or co-occurring behavioral health needs.
- Strong written and verbal communication skills.
- Ability to maintain complete, accurate, and timely clinical records.
- Ability to work effectively as part of a multidisciplinary behavioral health team.
Preferred Qualifications
- Previous outpatient behavioral health experience.
- Experience working within an OAR 309-019 behavioral health program.
- Experience with Oregon Medicaid behavioral health documentation.
- Experience using electronic health records (EHR).
- Experience working with co-occurring mental health and substance-use needs.
- Experience providing trauma-informed and person-centered services.
- Oregon professional licensure or associate registration when applicable to the candidate’s discipline.
