Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic team at a leading Skilled Nursing Facility. In this vital role, you will provide high-quality nursing care to residents, ensuring their comfort, safety, and well-being. Your expertise will support the interdisciplinary team in delivering personalized care plans, managing complex health conditions, and promoting optimal recovery and quality of life. Shifts: 12hrs 7a-7p or 7p-7a
Responsibilities
- Deliver comprehensive nursing care to residents, including medication administration, wound care, and vital signs monitoring.
- Conduct thorough patient assessments, document medical histories, and update electronic health records (EHR) systems accurately.
- Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
- Monitor residents for signs of acute pain or deterioration, providing timely interventions and triage as needed.
- Educate residents and their families on health management strategies, medication safety, and discharge planning.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN).
- Proven experience in long-term care settings such as nursing homes or assisted living facilities.
- Experience with EMR/EHR systems like Matrix is highly desirable.
- Demonstrated competence in critical care procedures including ventilator management, infection control practices, and acute pain management.
- Ability to perform physical examinations, vital signs assessment, specimen collection/processing, and basic life support (BLS).
- Excellent communication skills with the ability to work effectively with individuals with disabilities or cognitive impairments such as Alzheimer’s disease or dementia.
