Responsibilities
- Deliver direct patient care by monitoring vital signs, assisting with activities of daily living, and supporting medication administration in accordance with physician orders.
- Monitor and respond appropriately to seizure activity by following individualized seizure protocols, documenting incidents accurately, and ensuring client safety during and after episodes.
- Maintain accurate medical documentation and complete required reports in compliance with agency, DDA, and state regulations.
- Transport and drive clients safely to medical appointments, community outings, employment programs, recreational activities, and other approved destinations.
- Support individuals with achieving Person-Centered Plan (PCP) goals by promoting independence, community integration, social engagement, and skill development.
- Provide Direct Support Professional (DSP) services to individuals with intellectual and developmental disabilities in home and community-based settings.
- Assist clients with behavioral support plans, communication needs, emotional regulation, and daily routines while maintaining dignity and respect.
- Support patients with developmental disabilities, seizure disorders, memory care requirements, hospice needs, and other complex conditions through compassionate caregiving.
- Collaborate with nursing staff, case managers, guardians, and interdisciplinary teams to track patient progress and communicate changes in condition effectively.
- Follow all safety protocols related to infection control, medication administration, seizure precautions, medical procedures, transportation safety, and patient privacy standards.
Skills
- Proven experience working in assisted living facilities, nursing homes, hospitals, home care environments, or DDA community-based programs.
- Knowledge and experience supporting individuals with seizure disorders, including seizure recognition, documentation, safety precautions, and emergency response procedures.
- Strong knowledge of anatomy and physiology coupled with experience in medical terminology and documentation practices.
- Skilled in vital signs measurement, medication administration, catheter care, and tube feeding techniques.
- Ability to work effectively with individuals with intellectual or developmental disabilities while supporting individualized PCP outcomes.
- Valid driver’s license with a clean driving record and ability to safely transport clients in agency or personal vehicles as required.
- Excellent communication skills to coordinate seamlessly with healthcare teams while maintaining patient confidentiality under HIPAA guidelines.
- Experience in hospice care, memory care programs, behavioral health support, choking or seizure protocol is highly valued.
