Overview:
We are seeking a compassionate and organized Resident Case Manager to join our team. The ideal candidate will coordinate services, advocate for clients, and ensure quality care and support. This role requires excellent communication and problem-solving skills to help clients navigate complex healthcare or social service systems.
Key Responsibilities:
- Assess client needs and develop individualized care or service plans
- Coordinate care and services across multiple providers or community resources
- Monitor and evaluate client progress and update plans accordingly
- Advocate for client needs and support access to services
- Maintain accurate documentation and case files
- Collaborate with medical, social service, and community support teams
- Ensure compliance with regulations, policies, and best practices
- Organize transportation to/from medical appointments
- Assist in training staff/caregivers with medication management, medication administration, and documentation
Qualifications:
- Associate's or Bachelor's degree in Nursing, Social Work, Psychology, or related field (preferred)
- Prior experience in case management, social services, or healthcare coordination
- Strong organizational, communication, and interpersonal skills
- Proficiency with case management software and documentation
- Knowledge of community resources and service providers
- Valid Drivers License and transportation- REQUIRED
