Compassionate Comfort Care: Provide specialized personal care that prioritizes client comfort, including repositioning pressure relief and maintaining meticulous skin integrity.
Family-Centered Support: Offer a calming, empathetic presence to loved ones; provide emotional support and clear communication to help family members navigate the care process.
Gentle Personal Care: Assist with Activities of Daily Living (ADLs) such as bathing, dressing, and oral hygiene with the specialized gentleness and respect required for hospice clients.
Pain & Symptom Observation: Monitor the client closely for signs of discomfort, restlessness, or changes in breathing, and immediately report findings to the Director of Nursing and the hospice team.
Post-Mortem Care: Provide respectful and dignified post-mortem care in accordance with the family’s wishes and the agency’s clinical standards.
Environmental Tranquility: Maintain a quiet, clean, and organized bedside environment to promote a sense of peace for the client and a welcoming space for visitors.
Nutrition & Hydration: Provide specialized assistance with intake, offering small sips or bites as tolerated and managing mouth care to ensure continuous comfort.
Care Plan Adherence: Follow the personalized Plan of Care strictly, ensuring all comfort measures are implemented consistently.
Detailed Documentation: Maintain thorough and accurate records of care provided, vitals (as required), and any emotional or physical changes.
Current unrestricted HHA/CNA license in the state of Indiana.
Certified CPR and negative TB skin test or chest x-ray.
Minimum of one (1) year of documented experience; direct experience in hospice or palliative care is highly preferred.
Exceptional emotional intelligence, empathy, and professional composure.
Licensed driver with valid automobile insurance and reliable transportation.
Physical Ability: Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Must be able to safely lift and transfer up to 50 pounds.
