Job Summary:
- A Licensed Physical Therapist professional who provides coordinated care to patients, evaluates patients and restores or maintains patient well-being according to the plan of care
- Evaluate and provide a treatment plan for a patient with physical, psycho- social and/or cognitive impairments
- Provide intervention to the deficit and elicit responses
- Collaborates with interdisciplinary team
Job Qualifications:
- Education: Bachelor of Science in Physical Therapy
- Licensure: Current Texas State License as a Physical Therapist Assistant
Experience:
- 1 year of minimum experience as a Physical Therapist in Home Healthcare or Acute care or Rehabilitation setting
Skills:
- Therapy skills as defined as generally accepted standards of practice.
- Good communication and interpersonal skills.
- Proof of current CPR and Hepatitis profile.
- Knowledge of durable medical equipment
- Spanish is a Plus
Essential Functions:
- Perform initial and ongoing clinical assessments to identify patient needs, determine the level of function at appropriate time points and the appropriateness of patient for Home health
- Develop and revise a plan of care in consultation with the physician and other care team members
- Provide therapeutic treatments, evaluate need for and use of adaptive equipment and environmental modifications, fabricate and maintain adaptive equipment and hand splints as applicable
- Evaluate outcomes of care with appropriate discharge planning.
- Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
- Accept clinical assessments that are consistent with education and competence to care for patients.
- Supervise PTA as needed
- Complete appropriate documentation in a timely manner
- Meet mandatory continuing education requirements of the agency/license board
- Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate
- Demonstrate commitment, professional growth, and competency
- Promote Agency philosophy and administrative policies to ensure quality of care
