Meaningful work that directly impacts patients and families
Provide personal care to hospice patients in their homes and care facilities
Assist with bathing, grooming, dressing, toileting, and other activities of daily living
Provide repositioning, mobility assistance, and comfort-focused care
Observe and promptly report changes in a patient’s condition to the assigned nurse
Follow each patient’s individualized plan of care
Maintain accurate and timely visit documentation
Offer compassionate emotional support to patients and families
Communicate effectively with nurses and other interdisciplinary team members
Maintain patient dignity, privacy, safety, and comfort
Current Pennsylvania Certified Nursing Assistant certification
Hospice, home health, long-term care, or personal-care experience preferred
Valid driver’s license, reliable transportation, and automobile insurance
Ability to travel throughout the Wyoming Valley service area
Compassionate, dependable, and comfortable working independently
Strong communication and organizational skills
Commitment to providing respectful and exceptional end-of-life care
