Jefferson County Board Developmental Disabilities (JCBDD) is seeking a Service Support Administrator (SSA)

Requirements:
· Bachelor’s Degree: Special Education, Social Work, Mental Health, Rehabilitation, Psychology or closely related field
· Must be able to obtain and maintain SSA (Service Support Administrator) Certification through the Ohio Department of Developmental Disabilities
· Documented experience working with persons with Developmental Disabilities
· Must possess a Valid Driver’s license.
Highlights of the Duties:
· The SSA coordinates services and supports that ensure health and welfare within an individual’s assigned budget through the Individual Service Plan (ISP) planning process; this includes, but is not limited to, development of the ISP with the person and their team at least annually, completing all necessary assessment, writing the Annual ISP based on what is assessed as important to and important for a person and completing all necessary revisions to the plan when there are changes in the support needs.
· Assures that services are effectively coordinated for the individual and delivered by providers as outlined in the ISP. Conducts reviews of services delivered and incident reports to assure effectiveness and continuity of service delivery;
· Identifies resources available to the individual and matches them with the individual’s assessed need and expressed desires. Works with individuals to develop a budget that matches available resources to needs/desires;
· Assure that individuals are connected with appropriate community resources and funding to meet their needs;
· Maintain individual records and reports in a timely and accurate manner including but not limited to service delivery documentation, progress notations, periodic reviews, behavior support plans, emergency medical authorizations,
· Negotiates complex system/support issues between programs, contract agencies, community resources and selected providers;
· Assures Medicaid eligibility is maintained and protocols are completed for waiver recipients;
· Assures Medicaid eligibility is maintained
Skills:
· Knowledge in Microsoft office and ability to navigate computer database programs.
Typical Schedule:
Workdays would be Monday through Friday excluding holidays.
Hours 8:00 to 4:00 with flexibility to accommodate people supported by the agency
