RN Case Manager
REPORTS TO: DON/ADON
POSITION SUMMARY:
The purpose of the RN case manager to serve as case manager for Medicaid patients and help maintain stabilization, delay/prevent deterioration of health status, manage chronic conditions, and/or improve health status for patients while performing OASIS start of care, transfer, resumption, and discharge.
QUALIFICATIONS:
- License, required
- 2+ year nursing experience, required
- 1+ year Home Health nursing experience, required
- Case manager experience, preferred
- Must demonstrate excellent interpersonal skills; both written and verbal, time management, and Microsoft Office skills
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
- Completes start of care OASIS for patients to begin home health services as well as regular re-certifications.
- Initiates the plan of care which establishes patient-centered goals as approved by physicians.
- Responsible for the completion of all home health documentation including admissions, re certifications, discharges, and all related OASIS documentation.
- Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
