The ADON serves as a key clinical leader and may assume delegated responsibilities of the Director of Nursing as assigned. This position also offers a structured pathway toward Director of Nursing advancement based on performance and organizational growth.
Key Responsibilities
Clinical Operations & Oversight
- Assist in overseeing daily nursing operations to ensure efficient case management and appropriate staffing levels.
- Review and evaluate case referrals, home visit findings, and patient assessments to determine appropriate care needs.
- Assist in developing, reviewing, and updating individualized Plans of Care based on patient medical history and clinical presentation.
- Verify physician orders and ensure services are authorized and compliant.
- Coordinate interdisciplinary care efforts with physicians, therapists, social workers, and other healthcare professionals.
- Periodically review patient diagnoses, medications, treatment interventions, and clinical progression to ensure quality and effectiveness of care.
Staffing, Leadership & Productivity Management
- Support management of clinician schedules and visit distribution to maintain operational efficiency.
- Assist in maintaining optimal staffing levels aligned with patient acuity and agency growth.
- Provide mentorship, coaching, and structured performance feedback to nursing staff.
- Participate in interviewing, hiring, onboarding, and orientation of new clinical personnel.
- Foster a positive, accountable work culture emphasizing patient-centered care.
- Support and help maintain clinical productivity benchmarks as defined by agency standards.
- Assist in monitoring visit utilization and frequency alignment with Plan of Care and PDGM expectations.
Documentation, EMR & Regulatory Compliance
- Oversee documentation review processes to ensure accuracy, completeness, and timeliness.
- Ensure compliance with HIPAA regulations and confidentiality standards.
- Manage and monitor EMR/EHR systems to ensure seamless documentation and data integrity.
- Conduct regular audits of medical records and documentation processes.
- Monitor OASIS accuracy and submission timeliness.
- Support adherence to Medicare Conditions of Participation and regulatory standards.
- Assist with survey readiness and state/federal audit preparation.
Quality Metrics & Performance Reporting
- Support tracking and reporting of key clinical performance indicators including:
- Hospitalization and re-hospitalization rates
- OASIS accuracy and timeliness
- Visit utilization and productivity
- Documentation completion timelines
- Care coordination performance
- Medicare reimbursement optimization
- Participate in Continuous Quality Improvement (CQI) initiatives.
- Assist the DON and Administrator in preparing clinical dashboards and operational reports.
- Identify performance trends and assist in implementing corrective action plans when necessary.
Education & Professional Development
- Lead or assist in staff training on:
- Documentation standards and OASIS accuracy
- Medicare compliance
- EMR system optimization
- Safety protocols and best clinical practices
- Promote ongoing professional development opportunities.
- Evaluate clinical competency of staff at least annually.
- Encourage professional growth and continuing education participation.
Qualifications
- BSN preferred; ADN with strong home health leadership experience considered.
- Active RN license in the State of Ohio (required).
- Minimum 3–5 years of home health experience.
- At least 1 year of supervisory or leadership experience.
- Strong knowledge of:
- Medicare Conditions of Participation
- OASIS documentation
- PDGM reimbursement model
- Care planning processes
- HIPAA regulations
- EMR/EHR systems
- Comprehensive understanding of medical terminology, clinical assessment, and care planning.
- Licensed driver with insured automobile per Ohio requirements.
- Current CPR certification.
- Strong organizational, analytical, and communication skills.
- Ability to manage multiple priorities in a fast-paced clinical environment.
Additional certifications in home health or hospice are a plus.
