The Admissions Nurse (RN) is responsible for conducting hospice
admission assessments, determining hospice eligibility in collaboration with the hospice physician/medical director, and initiating hospice services in accordance with physician orders, hospice regulations, and organizational policies. This role serves as a key clinical liaison among patients, families, referral sources, physicians, and the
interdisciplinary group (IDG). This position will support Scottsdale.
Core Responsibilities:
- Conduct comprehensive hospice admission assessments, including physical,
psychosocial, and symptom evaluations
- Determine hospice eligibility in collaboration with physician and/or medical
director and the interdisciplinary group
- Ensures admission diagnoses and supporting documentation align with
applicable Local Coverage Determinations (LCDs)
- Educate patients and families on hospice philosophy, services, and care plans.
- Obtain and verify physician orders, consents, and required admission
documentation
- Coordinate timely admissions from hospitals, facilities, homes, and other referral
sources
- Initiate plans of care and communicate patient needs to the interdisciplinary
group
- Ensure accurate, complete, and timely documentation in the electronic medical
record
- Collaborate with intake, case management, and clinical staff to ensure seamless
transitions of care
- Provide emotional support and compassionate communication to patients and
families during admission process
- Maintain compliance with Medicare hospice regulations, state laws, and agency
policies
- Participate in on-call rotation or after-hours admissions as required (if applicable)
- Other duties as assigned
Basic Qualifications/Requirements:
- Current, unrestricted Registered Nurse (RN) license in the state of employment.
- Minimum of 2 years of clinical nursing experience.
- Valid driver’s license and reliable transportation.
- Ability to work independently and manage multiple priorities.
- Strong assessment, communication, and documentation skills.
- Hospice, palliative care, or home health experience preferred.
- Knowledge of Medicare hospice eligibility and regulations.
- Experience with electronic medical records (EMR) systems.
- Applicants must be legally authorized to work in the United States.
