Provide direct patient care in accordance with State Nurse Practice Act and hospice standards
-
Perform comprehensive patient assessments and support individualized plans of care
-
Deliver skilled nursing services focused on comfort and quality of life

Administer and monitor pain and symptom management interventions
-
Educate patients and families on diagnoses, disease progression, and care plans
-
Document patient care accurately and timely in the electronic medical record (EMR)
-
Collaborate with the interdisciplinary care team to ensure coordinated hospice care
-
Evaluate patient response to treatment and adjust care as needed
-
Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program

Current LPN/LVN license in the state of practice

Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)

Current BLS certification

Strong clinical assessment, communication, and organizational skills

Valid driver’s license, reliable transportation, and proof of insurance

Understanding of hospice philosophy and end-of-life care

Experience in hospice or palliative care

Familiarity with home-based care and EMR systems
