Advance Performance Pain & Wellness Center is looking for a dynamic Physical Therapist to join our growing team. We are seeking an experienced candidate with strong manual therapy skills who is committed to working in an engaging, collaborative, team-oriented clinic. This role requires a strong Orthopedic and Sports Medicine background with prior experience working in an integrated setting a plus.

Role Description:
- Provides direct patient care, evaluates functional needs and outcomes, and collaborates with other providers in the clinic. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.
- Administers physical therapy treatments such as patient exercise using equipment and objective functional testing.
- Employs treatment strategies that utilize proven treatment strategies aimed at achieving patient outcomes and patient retention.
- Conducts therapy practice within defined standards of care while maintaining all ethical, legal and professional responsibilities.
- Directs the assistance of support staff to achieve patients’ functional goals.
- Communicates effectively with DOR, teammates and facility staff.
- Maintains excellent documentation with evaluations, treatment goals, and plans. Regularly updates documentation with confidentiality while maintaining all prescriptions and signatures.
REQUIREMENTS / EXPERIENCE:
- Licensed Physical Therapist in Illinois
- Orthopedic, Sports Medicine, Physical & Rehabilitation Medicine experience of 2+ years preferred.
- Doctorate preferred.
- Strong patient care and manual therapy skills.
