SUMMARY OF POSITION:
Responsible for working with the Access department to take referrals, working with clients, case
managers, Doctors and or primary RNs to facilitate the start of services. Collect data regarding
patients and make plans accordingly to complete the initial assessment of patient and family to
determine needs, initiate the Plan of Care, and provide nursing care to promote comfort of the
patient and family. Perform PRN visits to current hospice patients in the home setting as
needed. Provide Hospice RN Case Management for a routine home care hospice patient as
needed.
MINIMUM REQUIREMENTS
▪ Graduate of an accredited school of nursing.
▪ One to two years of recent acute care experience in an institutional setting.
▪ Ability to represent the organization professionally at all times.
▪ Ability to be flexible and adaptable to changing workload hours.
▪ Current professional licensure in New York State, and valid driver’s license with driving
record which meets standards outlined in Policy: Qualifications of Hospice Personnel.
▪ Bachelor’s degree, with one year of home health care experience. Registered nurses with
less than a Bachelors degree will be considered based on previous experience.
▪ Management experience is not required but supervisory skills preferred
▪ Prolonged or considerable walking or standing. Able to lift, position and/or transfer
patients. Able to lift supplies and equipment. Considerable reaching stooping, bending,
kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
▪ Excellent observation, verbal and written communication skills, problem solving skills,
customer service skills
Pay: $77,500.00 - $80,500.00 per year
