Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine individualized care needs.
- Develop and implement personalized case management plans that align with clinical guidelines, hospital policies, and patient preferences.
- Collaborate closely with healthcare professionals across disciplines such as physicians, therapists, social workers, and discharge planners to coordinate care effectively.
- Manage utilization review processes by evaluating medical necessity, supporting appropriate resource allocation, and ensuring compliance with health regulation policies.
- Facilitate discharge planning by coordinating post-acute care services, home health arrangements, hospice care options, and outpatient follow-ups.
- Maintain accurate and detailed medical records using Electronic Health Records (EHR) systems while adhering to HIPAA regulations to protect patient confidentiality.
- Engage in patient education initiatives to enhance understanding of treatment plans, medication management, and health promotion strategies.
Skills
- Proven experience in critical care environments such as ICU or Level II/Level I Trauma Centers with a strong understanding of trauma and emergency medicine protocols.
- Extensive knowledge of hospital medicine, infectious disease care, geriatrics, behavioral health, and medical-surgical nursing practices.
- Skilled in clinical case management with proficiency in patient assessment, diagnostic evaluation, and utilization management.
- Familiarity with Wellsky EMR systems previously (Kinnser) for efficient documentation and medical records management.
- Excellent client communication skills with the ability to collaborate effectively with diverse healthcare teams.
- Strong understanding of health regulation policies including HIPAA compliance and medical terminology.
- Ability to manage multiple complex cases simultaneously while maintaining attention to detail and delivering high-quality patient service.
- Experience in outpatient clinics or post-acute care settings is highly desirable for this role.
