American Traveler is seeking an experienced RN for a Home Health Case Manager position in Sioux Falls, SD, requiring Epic proficiency and on-call weekend availability.

Home health setting with a post-acute care focus

Monday through Friday day shift schedule with 8-hour shifts

On-call coverage required every 3rd–4th weekend and one day per week

One holiday coverage required

Uses Epic EMR

13-week contract

Active SD or Compact RN license, currently in hand (no pending licenses accepted)

Current BLS certification required

Minimum 1 year of relevant RN experience required — no exceptions

Valid driver's license required

Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, each including dates of employment and eligibility for rehire

Candidates must reside more than 80 miles from the facility to qualify for travel pay rates; local candidates within 80 miles are accepted at a different rate
