LPN Medication Care Manager
The Medication Manager is under the direct supervision of the Medical Director. Providing oversight and monitoring of client medication; education regarding medications and appropriate resources and supportive case management services.
Responsibilities
Maintain accurate medical records and update patient information as necessary by documenting medications not administered on the MAR and the reason for not being administered.
Experience in medication administration, including oral, topical and injectable routes
Managing distribution, storage, and disposal of all controlled substances.
Triaging and referring patients for medical treatment based on their physical or mental health presentation. Assessing patients' eligibility for care and reviewing medical records
Providing oversight and training to Residential Managers or Medication Technicians who distribute medication to the clients enrolled in the substance abuse program.
Performing medication administration responsibilities while adhering to all safety principles.
Experience in medication administration, including oral, topical, injectable, and intravenous routes
Possess knowledge in managing intravenous medications under the supervision of an RN, APRN or MD
Demonstrate a thorough knowledge of anatomy and physiology to effectively understand the effects and interactions of different medications
Performing routine physical assessments, measuring and documenting vital signs such as blood pressure, temperature, height, weight, pulse, and oxygen saturation
Providing patient education as directed by a prescribing healthcare provider.
Identifying and reporting any adverse patient medication reactions or complications immediately to the healthcare provider
Serving as a liaison for communication between hospitals, healthcare providers, clinical or management teams, patients, and family members
Participating in data collection, medication audits, and other performance improvement activities related to medication administration.
Reordering and replacing medications and medication administration supplies, as needed, to perform routine job responsibilities.
Collaborate with healthcare professionals, pharmacies or external clinics ( including methadone programs) to develop comprehensive medication plans for each client.
Ensuring the confidentiality and rights of clients, the confidentiality of the health systems and department documents, and adhering to HIPAA Regulations.
Keep all licenses and certifications up to date and in good standard

Qualifications
Current CPR and First Aid Certificate. Current Maryland LPN License. Pass a criminal background check and CPS check.
Education/ Experience
Associate or Bachelor in Nursing. Three years’ related experience and or training: or equivalent combination of education and experience. Experience in medication administration, including oral, topical, injectable, and intravenous routes. Possess knowledge in managing intravenous medications under the supervision of an RN, APRN, or MD
