TITLE: Social Worker (MSW)
DEPARTMENT: Patient Care Services
FLSA STATUS: Hourly – FULL TIME
NATURE AND SCOPE OF POSITION : In accordance with NORTHLAND HOSPICE Policy and Procedures, the Social Worker serves as a member of the Interdisciplinary Care Team to provide social work services to the terminally ill and their families through a systematic process of assessment and reassessment and consultation regarding the bio-psychosocial needs of the hospice patient/family as needed, and facilitates support services.
ESSENTIAL DUTIES:
- Perform bio-psychosocial assessments and reassessments that encompass the total well-being of the patients and family.
- Develops an effective plan of treatment in collaboration with the referring, attending physician, medical director and multidisciplinary team.
- Documents all pertinent information appropriately, accurately, and timely.
- Provide individual and family and group counseling – If applicable
- Provide education to patients and family/caregivers about coping skills, hospice and palliative care philosophy, and nonpharmacological symptom management strategies.
- Providing in-services to other service providers and organizations.
- Discharge planning, oversight, coordinating care, and helping clients navigate systems.
- Facilitate advance care planning.
- Crisis intervention.
- Mediating conflicts within families, between clients and the interdisciplinary team, and between service organizations.
- Participating in interdisciplinary team meetings, care planning, and ethics consultations.
- Advocating on behalf of the patient and family.
- Provide bereavement assessment and follow up.
- Consult with other team members and participate in weekly Interdisciplinary Case Conferences.
- Assist the team in establishing an appropriate Care Plan to meet patient and family needs.
- Make periodic visits as required by the care plan.
- Participate in family conferences.
- Assist patient/family in accessing appropriate community resources.
- Serve as an advocate for the terminally ill, their family/loved ones and the bereaved.
- Provide bereavement support on one-to-one basis for hospice family members and community members who request services.
- Participate in public speaking opportunities, as it relates to grief support services and grief process. Assumes compliance with State and Medicare regulations

- Participation in community and company activities is not mandatory but highly recommended, and will be reflected in annual performance reviews
- Maintain accurate current records and documentation of all services provided in a manner consistent with Vista Hospice Policy and Procedures and Social Work and/or Counseling standards of practice as applicable.
- Ensures compliance with Medicare CoPS for psychosocial, bereavement, volunteer, and spiritual services.
- Maintain compliance with HIPAA requirements as per organization policies.
- All other duties as assigned.
EDUCATION AND/OR EXPERIENCE: * Masters Degree in Social Work from an accredited School or Department of Social Work.
- State of Arizona Board of Behavioral Health license required if licensed as an LMSW or LCSW. If no current license at time of employment, must be eligible for licensure. Social Work diploma required if not licensed.
- Previous hospice experience preferred
- Minimum of one year previous social work experience
- Demonstrate sensitivity to the needs of patients/families in stressful situations and tolerance for various individual family life styles.
- Knowledge of grief process for clients and staff members.
- Demonstrates sensitivity and respect to colleagues.
- Organizational and computer skills required.
- TB clearance upon hire and then annually.
- Pass drug screen upon hire.
- Complete NHPC employment process.

CERTIFICATES/LICENSES/REGISTRATIONS
- Reliable transportation, current driver’s license, and current auto insurance
- Graduate first available Northland Hospice Volunteer Training Program
- Current TB test required and maintained annually
- Current and valid Fingerprint Clearance Card
