Position Summary
The Nurse provides clinical support and care coordination for patients. The Nurse works collaboratively with physicians, facility staff, families, home health agencies, hospice providers, and ancillary services to ensure high-quality, patient-centered care.
Essential Duties and Responsibilities
Patient Care Coordination
- Coordinate care for patients receiving primary care services.
- Assist with implementation and follow-up of treatment plans.
- Facilitate referrals, diagnostic testing, therapy services, and home health services.
Clinical Support
- Perform nursing assessments as assigned.
- Obtain and document vital signs and clinical information.
- Review laboratory and diagnostic results for provider follow-up.
- Assist providers with Annual Wellness Visits (AWVs), chronic disease management, and routine follow-up care.
- Support medication reconciliation and patient education.
Documentation
- Maintain accurate and timely documentation within the electronic medical record.
- Ensure patient information is complete and compliant with regulatory standards.
- Assist with care plans and clinical documentation requirements.
- Maintain confidentiality of protected health information (PHI).
Communication
- Serve as a liaison between providers, patients, families, facilities, home health agencies, pharmacies, and specialists.
- Communicate changes in patient condition to the appropriate provider.
- Participate in team meetings and clinical workflow initiatives.
- Foster positive relationships with community staff and resident representatives.
Compliance and Quality
- Follow all HIPAA, OSHA, and company policies.
- Comply with state nursing regulations and facility-specific requirements.
- Participate in quality improvement initiatives and staff training.
- Maintain current licensure and required continuing education.
Qualifications
- Current Alabama nursing license in good standing (LPN or RN).
- Valid driver's license and reliable transportation.
- Experience in primary care, geriatrics, long-term care, home health, hospice, or assisted living preferred.
- Strong communication, organization, and time management skills.
- Proficiency with electronic medical records.
Physical Requirements
- Ability to travel between patient care sites.
- Ability to lift up to 25 pounds.
- Ability to sit, stand, walk, bend, and perform routine clinical duties throughout the workday.
Work Schedule
- Full-time or part-time available.
- Travel to assisted living facilities, independent living communities, and patient homes within the service area.
