About the Role:
As our new MDS Coordinator, you will be responsible for overseeing and facilitating resident assessments, ensuring accurate and timely completion, and transmitting assessments to the Massachusetts Department of Health. You will also coordinate resident care plans, initiate Medicare coverage, and stay up-to-date on changes in Medicare coverage and required documentation. Additionally, you will educate and support nursing staff and other departments on assessments, care planning, and the MDS process.
Responsibilities:
- Oversee and facilitate the completion of resident assessments in accordance with Federal and State regulations
- Establish assessment schedules and ensure that assessments are completed accurately and in a timely manner
- Transmit assessments to the Massachusetts Department of Health
- Coordinate resident care plans in accordance with regulatory requirements, ensuring effective delivery of care
- Initiate Medicare coverage for newly qualified residents and issue denial letters when necessary
- Stay up to date on changes in Medicare coverage and determine required documentation for reimbursement
- Coordinate the assessment, planning, implementation, and evaluation of resident care through the MDS process
- Facilitate care conferences with the interdisciplinary team
- Coordinate, complete, and review Minimum Data Sets (MDS)
- Educate, train, and support nursing staff and other departments on assessments, care planning, and the MDS process
- Ensure all current MDS updates are implemented and staff are properly in-serviced
- Conduct regular resident interviews as required by MDS
Requirements:
- Must be a licensed Massachusetts RN or LPN
- Minimum 1 year of experience as an MDS Coordinator
- Strong communication, organizational, and assessment skills
