The Care Coordination Case Manager serves as a vital link between members, medical providers, behavioral health teams, and support services. This role helps coordinate care, manage member health-related needs, support medical and behavioral health appointments, and ensure members receive the resources and services necessary to support their recovery, wellness, and long-term success
This role is ideal for someone who:

- Coordinating member care and communicating member needs across clinical and healthcare teams
- Assisting members with medical services, referrals, appointments, and healthcare navigation
- Maintaining accurate records, documentation, and member medical information
- Supporting member check-in, scheduling, and other care coordination activities
- Assisting with telehealth, tele-med, and tele-psych appointments
- Helping ensure compliance with HIPAA, OSHA, and organizational requirements
- Providing compassionate support to members while promoting engagement in care and services

- Members receive timely, coordinated, and supportive healthcare services
- Appointments, referrals, and care coordination activities are completed efficiently and accurately
- Member records and documentation are maintained with accuracy and confidentiality
- Strong collaboration exists between members, providers, and internal care teams
- Members remain engaged in services and experience a positive care experience
- Quality standards, compliance requirements, and program KPIs are consistently achieved

- High school diploma or GED
- Behavioral Health Technician (BHT) designation
- Basic knowledge of patient care, medical terminology, healthcare documentation, and medical support services
- Strong communication, organization, critical thinking, and problem-solving skills
- Ability to work effectively both independently and within a multidisciplinary team
- Intermediate proficiency with Microsoft Office and electronic health record systems
- CPR/First Aid certification and crisis de-escalation certification
- Must be at least 21 years of age
- Medical Assistant certification preferred
- One to two years of healthcare, behavioral health, care coordination, or case management experience preferred
- Bilingual (Spanish) skills preferred
- Experience supporting telehealth, medical referrals, or member care coordination programs
