Your Role
We’re looking for a passionate and skilled Home Health Speech Language Pathologist (SLP) to join our team. In this role, you’ll evaluate, diagnose, and treat patients with speech, language, cognitive, and swallowing disorders, helping them improve communication and quality of life. You’ll collaborate with our interdisciplinary care team to ensure every patient receives comprehensive and effective care.
What You’ll Do
- Conduct evaluations and develop individualized treatment plans tailored to each patient’s goals
- Provide therapy for speech articulation, language development, voice, and swallowing disorders
- Educate patients and caregivers on exercises and strategies to support progress at home
- Monitor patient outcomes and update care plans as needed
- Document assessments, visits, and progress accurately in compliance with Medicare and agency standards
- Collaborate closely with nurses, physicians, and other therapists to coordinate care
What We’re Looking For
- Active Florida Speech Language Pathologist (SLP) license (required)
- Home health experience preferred
- Familiarity with Kinnser/WellSky EMR a plus
- Excellent communication, organization, and time management skills
- Compassionate, patient-focused, and professional demeanor
- Ability to work independently and manage a flexible schedule
