Position Summary
The LVN or RN provides nursing direction and leadership to the nursing staff and delivers direct patient care as needed. This position works under the guidance of the Director of Nursing (DON) and supports clinical excellence and regulatory compliance.
Key Responsibilities
- Supervise Nurse Aides and Medication Aides; provide coaching and discipline as appropriate.
- Receive and evaluate reports on resident condition and ensure appropriate resources are in place to meet care needs.
- Conduct physical rounds within the assigned area to assess the quality of care, intervene when necessary, and interact with residents and caregivers.
- Provide direct nursing care, including resident assessments, communication with attending physicians or practitioners, and implementation of nursing interventions.
- Accurately document resident assessments, care provided, and clinical communications in accordance with facility and regulatory standards.
- Ensure adherence to safe work practices and infection control protocols.
- Communicate empathetically with residents and families regarding changes in condition and care concerns, while ensuring resident rights are respected.
- Assist with Continuous Quality Improvement (CQI) initiatives.
- Other duties as assigned by the Assistant Director of Nursing/Director of Nursing.
Required Qualifications
- Current Licensed Vocational Nurse (LVN) or Registered Nurse (RN) license in the applicable state, or a multistate compact nursing license with authorization to practice in the applicable state.
- Prior experience in healthcare, long-term care (LTC), or skilled nursing facility (SNF) settings.
- Effective communication and interpersonal skills.
- Current BLS/CPR certification.
- Ability to lift at least 51 pounds and physically assist residents as needed.
- Ability to pass a criminal background check and drug screening.
