You'll partner directly with our Primary Care Provider on clinical decisions, coordinate closely with skilled nursing facilities and hospitals as participants transition between settings. What You'll Do
- Manage a caseload with a strong care management focus alongside clinical care
- Participate in health assessments and examinations as part of our Intake and Assessment Team
- Develop and contribute to comprehensive nursing treatment plans for each participant
- Provide and coordinate nursing care at our Day Health Center, with occasional home visits as directed by the physician
- Manage minor illnesses, injuries, and stable chronic conditions, and initiate preventive and rehabilitative nursing procedures
- Evaluate acute illnesses in collaboration with each participant's primary care provider
- Coordinate referrals and follow-up care with outside healthcare providers, and with hospitals and skilled nursing facilities as participants move between settings
- Teach, supervise, and counsel participants and family members on nursing care needs at home
- Consult with and educate health workers and Home Care staff as needed
- Review participant charts to ensure medical plans are carried out
- Provide coverage in our Primary Care Clinic, Monday through Friday, with occasional backup coverage for Saturday hours
What You Bring
- Current, active California RN license
- At least 2 years of nursing experience at the RN level
- Experience working with an elderly or geriatric population
- Demonstrated initiative, imagination, resourcefulness, and flexibility
- A genuine ability to work effectively as part of an interdisciplinary team; this role has a heavy teamwork component, and we want people who speak up and share their clinical judgment openly
- Interest in and willingness to work in a multilingual, multicultural environment
- Public Health Nurse Certificate preferred (or willingness to obtain one)
- Experience in public health nursing, acute care, or working with older adults preferred
- Computer literacy preferred
- Spanish or Cantonese fluency preferred
