We are looking for a Restorative Nurse (RN) who is passionate about helping residents regain and maintain their highest level of independence. This role focuses on supporting residents’ physical, mental, and emotional well-being through restorative nursing programs designed to maintain mobility, strength, and quality of life.
As the Restorative Nurse, you will play a key role in coordinating restorative care efforts, collaborating with therapy and nursing teams, and ensuring each resident’s care plan supports their highest practicable level of function.

Coordinate and oversee restorative nursing programs for assigned residents

Work closely with the Director of Nursing, therapy teams, and nursing staff to support restorative care goals

Participate in care planning and the MDS/RAI process related to restorative services

Evaluate residents’ functional abilities and identify opportunities to improve mobility and independence

Initiate therapy referrals and serve as the liaison between therapy services and nursing staff

Monitor resident progress and adjust restorative programs as needed

Educate residents, families, and staff on restorative care practices

Ensure restorative care policies and procedures are followed

Active Registered Nurse (RN) license in good standing

Experience working with elderly or disabled populations, preferably in long-term care

Understanding of restorative nursing programs and care planning

Strong leadership and communication skills
