Responsibilities
- Engage with community members to assess their health needs and provide tailored support and education.
- Participate as a driver in our transportation program to assure patients have access to their medical appointments.
- Facilitate access to healthcare services by guiding individuals through enrollment processes for programs like Medicare and Medicaid.
- Conduct outreach activities such as workshops, public speaking engagements, and health fairs to promote awareness of health resources.
- Utilize motivational interviewing techniques to encourage behavioral change and foster motivation for healthier living.
- Collect and record data related to community health trends, service utilization, and individual progress to support program evaluation.
- Collaborate with healthcare providers, social workers, and local organizations to coordinate care plans for clients with disabilities or chronic conditions.
- Educate community members on topics including nutrition, addiction counseling, medical terminology, and public health practices.
- Work closely with our volunteers and community partners to conduct monthly foot clinics.
Experience
- Prior experience in nursing, social work, or public health is highly desirable.
- Demonstrated ability to work effectively with individuals with disabilities or those facing complex social challenges.
- Strong communication skills with experience in public speaking or teaching roles.
- Familiarity with data collection methods and basic understanding of medical terminology.
- Knowledge of Medicare policies, nutrition education, and addiction counseling principles is a plus.
- Experience using motivational interviewing techniques to support behavioral change initiatives.
- Ability to build trust within diverse communities through empathetic listening and culturally sensitive approaches.

Pay: From $18.00 per hour
