South Mountain Post Acute is a 5-Star Skilled Nursing Facility in South Phoenix seeking a Licensed Physical Therapist to join our in-house therapy team.

What you’ll do:
- Evaluate patients within 48 hours of referral
- Develop and implement individualized treatment plans
- Supervise PTAs and rehab aides
- Collaborate with interdisciplinary care teams
- Participate in care conferences and discharge planning
- Document treatments and progress in EMR
- Coordinate adaptive and durable medical equipment
Qualifications:
- Active Physical Therapist license
- All experience levels welcome , new grads encouraged
