Hope and Health Direct Primary Care is seeking a dedicated, high-integrity Nurse Practitioner to join our practice. In the DPC model, we strip away the bureaucratic hurdles of traditional insurance-based medicine to focus on what matters most: the patient-provider relationship.

Primary Roles & Responsibilities
In accordance with the standard of care for Nurse Practitioners in the state of Texas, your duties will include:
- Comprehensive Care: Perform physical exams, diagnose and treat acute and chronic illnesses, and manage the overall health of a diverse patient panel.
- Clinical Judgment: Order, interpret, and manage diagnostic tests (labs, imaging) and develop evidence-based treatment plans.
- Prescriptive Authority: Prescribe medications and non-pharmacological therapies as authorized by the Texas Board of Nursing.
- Patient Advocacy: Provide thorough patient education and counseling on preventive health and lifestyle changes—a cornerstone of the DPC model.
- Care Coordination: Manage referrals and collaborate with specialists while maintaining the DPC "medical home" philosophy.
- Documentation: Maintain precise, timely, and honest electronic health records that reflect the high standard of care provided.
Qualifications
- Licensure: Active, unrestricted license as a Nurse Practitioner in the State of Texas.
- Certification: Board Certification (AANP or ANCC) in Family Practice or Adult-Gerontology.
- Experience: Previous experience in primary care or urgent care is preferred, but a demonstrated "self-starter" attitude is paramount.
- Technical Savvy: Comfort with modern EMR systems and a willingness to utilize technology to enhance patient access (telemedicine, secure messaging).
