Job Summary:
We are seeking a compassionate and proactive Healthcare Navigator to join our team at ThrYve Teams in Brooklyn, NY. As a key member of our organization, you will play a vital role in ensuring that every child has access to the support they need to advance and develop in a way that is tailored to their unique needs. With your exceptional communication and interpersonal skills, you will promote awareness of our services and resources within the community, distribute approved educational materials, and provide general information about our services and available resources.
Responsibilities:
Promote awareness of our services and resources within the community
Distribute approved educational and marketing materials
Attend community events, health fairs, and outreach activities
Provide general information regarding our services and available resources
Direct interested individuals to authorized staff for screening, eligibility review, and enrollment processes
Maintain records of outreach activities as required
Represent the organization professionally and ethically at all times
Willing to drive your own car preferred
Qualifications:
Strong communication and interpersonal skills
Ability to engage with community members in a professional manner
Experience in outreach, customer service, healthcare, or social services preferred
Ability to work independently and maintain professionalism in community settings
