The Harris Center for Mental Health and IDD (“The HARRIS CENTER”) is looking for a High Fidelity Wrap around Facilitator to join our Texas Family First Prevention Services (TXFFPS) team. Under the general direction of the High-Fidelity Wraparound Clinical Supervisor or designated supervisor for Texas Family First Prevention Services. The High-Fidelity Wrap Around Facilitator provides direct ongoing intensive case management services through High-Fidelity Wraparound evidence-based principles and practices. This position requires extensive collaborative working relationships with many community partners including Department of Family and Protective Services (DFPS) staff at the local and state level, and the Harris County Juvenile Justice Courts. This position has extensive travel to client’s homes or other field locations, when providing services.
Your role in Action:

Collaborates , monitors and coordinates with Department of Family and Protective Services (DFPS) Child Investigators and Family Based Social Services workers and Harris county Juvenile Justice Courts to provide the best behavioral health High Fidelity Wraparound Plan for the families.

Performs clinical assessments to identify service needs and progress with family treatment plans of care goals.

Participates in formulation of the family wraparound plans and assures wraparound plan is written and authorized with an LPHA signature for consumers on caseload

Ensures all services provided meet eligibility criteria and are authorized in accordance with agency and regulatory guidelines.

Provides skills training and psychosocial rehabilitation services that address the severe and persistent mental illness and other behavior health symptom related problems that interfere with the family functioning.

Provides interventions in response to crisis to reduce symptoms and avoid admission to a more restrictive environment.

Collaborates, monitors and coordinates with Department of Family and Protective Services (DFPS) Child Investigators and Family Based Social Services workers transfer to hospitals and outpatient services in accordance with treatment plan and recommendations ss it relates to crises services.

Educates family members regarding the youth and family needs (physical, social, and mental health) as identified by the plan of care.

Performs activities to assist youth and families in gaining and coordinating access to necessary care and services within the community based on mental, physical and other social service needs.

Provides information on identified behavioral health issues, the importance of medications and treatment compliance, and other clinical related information to individual, family, and community as needed.

Monitors consumer progress with treatment goals

Monitors current compliance with eligibility requirements for ongoing treatment (CANS / ANSA, financial assessment, current diagnosis) using the monthly SAI report and other available information

Meets divisional productivity standards as outlined in the mental health division’s operational guidelines

Performs related work as assigned. Adapts to changing work requirements.

Demonstrates ethical conduct in all clinical and professional activities

Complies with agency policies and procedures accrediting/regulatory bodies and performance contract documentation requirements

Attends and participates in team meetings with the clinical team
What qualifications you will need:
Education:
-
A minimum of Bachelor’s Degree in the Human Services field (psychology, sociology, etc.)
Experience:
-
Minimum of one (1) year experience working in the mental health field, preferred.
Knowledge Skills and Abilities:

Basic understanding of mental illness, common signs and symptoms and able to identify difficulties with coping in community settings.

Strong communication skills, listening and interpersonal skills above average,
-
written documentation is clear, concise, well- constructed and grammatically correct.

Able to work collaboratively with others in a teamwork environment.

Effective time management and organizational skills.

Able to execute standard word processing software on the computer
