Core Primary Care | Sugar Land, Medical Center (initial training in TMC) | Full-time, Monday to Friday, 9 to 5 | $18 to $20/hour
Most people in healthcare are never taught how to help patients become healthier. We will teach you.
Get paid to learn about chronic diseases and the health behaviors that prevent them. Work alongside our clinicians and program manager to build a panel of patients who will know your name and call you first. If you have ever wanted to do more than take vitals and move to the next room, this is that job.
The Role
- Carry your own panel of patients living with chronic conditions such as diabetes, high blood pressure, and heart disease.
- Talk with them every month, watch the readings coming in from the cuff at home, and work beside their clinician on the same chart.
- Most chronic care management gets outsourced to a call center that logs minutes and changes nothing. We are building the opposite, inside our own practice.
- We train you into the role, including how to actually help someone change. Most people in healthcare are never taught that.
Key Responsibilities
- Daily workflow: work the list our platform builds for you, including readings that crossed a line, patients who have gone quiet, and check-ins coming due.
- Monthly calls: real conversations with your panel, not scripts.
- Home monitoring: review readings each business day, help patients keep devices working, and reach out when readings stop coming.
- Escalation: When a patient reports new symptoms or complex concerns, route them directly to the clinician that same day in the patient’s own words—no independent clinical judgment required.
- Coaching: deliver our clinician-approved education on food, movement, sleep, and medicines, and reinforce the signed care plan.
- Documentation: every contact, written the same day it happens.
What We Measure
- Cadence: patients reached on schedule, with prompt outreach when a contact is missed.
- Integrity: honest, same-day documentation.
- Coaching quality: reviewed on recorded calls. Worth knowing before you apply, this role involves recorded and scored patient calls, for everyone in the role.
- Quality Impact: Patient progress on eCQMs (electronic Clinical Quality Measures), ensuring your panel stays up to date on essential preventative care and disease management goals.
Qualifications
- No specific background/license required: medical assistant experience (certified or not), an LVN license, a health coach certification, or a degree in a health-related field. Medical assistants are very much encouraged to apply.
- Bilingual: fluent English and Spanish.
- Phone presence: a warm, plain way of talking with people who are frustrated or scared.
- Honesty: complete integrity about documentation.
- Experience: no care-management experience required. We train you into this.

Pay: $18.00 - $20.00 per hour
