Affirmed Home Care is hiring a Full-Time Licensed Social Worker to support clients throughout Manhattan. This is a rewarding opportunity for a compassionate professional who wants to make a real impact by helping individuals and families safely age in place.
In this role, you will provide client-centered social work services, support families, coordinate resources, and ensure clients receive the care and support they need to maintain independence and quality of life.

What You’ll Do

Conduct psychosocial assessments of clients and families

Perform home visits to evaluate living environments

Develop and implement individualized care plans
-
Serve as the main point of contact for clients and families

Coordinate community resources and support services
-
Lead client and family meetings
-
Collaborate with clinical and interdisciplinary teams

Educate families on care plans and available resources

Ensure accurate documentation and compliance
What We’re Looking For

Active New York State LMSW or LCSW license

Master’s Degree in Social Work (MSW)

1+ year of experience in healthcare or social services (home care preferred)

Strong case management and care coordination skills

Excellent communication and interpersonal skills

Ability to manage caseloads in a fast-paced environment
