Job Summary
Azure Home Health is seeking a compassionate and dependable Home Health Aide (HHA) to provide high-quality personal care services to patients in their homes under the supervision of a Registered Nurse (RN) or licensed therapist. The ideal candidate is caring, reliable, and committed to helping patients maintain their independence, dignity, and quality of life.
Responsibilities
- Provide personal care including bathing, grooming, dressing, toileting, and oral hygiene.
- Assist patients with mobility, transfers, ambulation, and range-of-motion exercises.
- Help with meal preparation and feeding as directed in the care plan.
- Monitor and report changes in the patient's physical or emotional condition to the supervising nurse.
- Take and document vital signs as assigned.
- Assist with light housekeeping related to the patient's care.
- Maintain a safe, clean, and comfortable environment for the patient.
- Reinforce patient and family education provided by the clinical team.
- Complete accurate and timely documentation after each visit.
- Maintain patient confidentiality and comply with all agency policies and regulations.
- Attend required meetings and in-service training.
Qualifications
- Current Home Health Aide (HHA) certification or Certified Nursing Assistant (CNA) certification (as required by state regulations).
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to travel to patient homes within the assigned service area.
- Excellent communication and interpersonal skills.
- Ability to work independently while following the patient's care plan.
- Previous home health experience is preferred but not required.
Physical Requirements
- Ability to lift, transfer, and assist patients safely.
- Ability to stand, walk, bend, and kneel for extended periods.
- Ability to perform physical tasks associated with patient care.
