Position Summary:
The Licensed Vocational Nurse (LVN) provides direct nursing care and clinical support services to individuals in their homes under the supervision of a Registered Nurse (RN), physician, or authorized healthcare provider in accordance with California laws, regulations, physician orders, and organizational policies. The LVN supports health monitoring, medication administration, treatments, documentation, and care coordination while promoting person-centered care, safety, dignity, and quality outcomes within home and community-based settings.
This position primarily provides services within clients’ homes and may require travel between assigned service locations.
Position Scope:
The LVN serves as a field-based clinical support professional responsible for delivering in-home nursing services, monitoring health conditions, supporting implementation of care plans, maintaining documentation, and collaborating with individuals, families, caregivers, providers, and interdisciplinary teams. The LVN exercises professional judgment within the California LVN scope of practice, delegated nursing functions, physician orders, and established care plans. Work assignments, duties, and physical demands may vary depending on individual client needs, care plans, home environments, and operational requirements.
Key Responsibilities:
- Provide in-home LVN nursing care, including medication administration and ordered treatments
- Monitor, assess, document, and report patient conditions and vital signs
- Implement care plans and physician orders with accurate clinical documentation
- Perform approved clinical procedures (e.g., wound care, catheter care, mobility support)
- Promote infection control, safety, dignity, and person-centered care
- Collaborate with RNs, physicians, caregivers, and interdisciplinary teams
- Educate clients and caregivers on care, medications, and treatments
- Respond to changes in condition and escalate urgent issues appropriately
- Travel to client homes and provide independent care in community settings
- Ensure compliance with regulations, policies, and quality standards

Failure to comply with regulatory or organizational requirements may result in corrective action up to and including termination.
Confidentiality:
The LVN may have access to protected health information (PHI), confidential medical records, and sensitive personal information. Strict confidentiality must be maintained in accordance with HIPAA, California privacy laws, organizational policies, and professional ethical standards. Unauthorized disclosure or misuse of confidential information may result in disciplinary action up to and including termination.
Documentation Requirements:
Required documentation may include, but is not limited to:
- Medication Administration Records (MARs)
- Nursing notes and visit documentation
- Assessments and health monitoring records
- Incident reports and follow-up documentation
- Physician communication records
- Care coordination notes
- Treatment documentation
- Infection control documentation
- Electronic Health Record (EHR) documentation
- Additional documentation required by regulatory agencies, physicians, payors, or organizational standards
All documentation must be accurate, complete, timely, and submitted within required deadlines.
Certifications:
Required Upon Hire:
- Active California Licensed Vocational Nurse (LVN) License in good standing
- Current CPR/BLS certification
- Ability to obtain and maintain all required background clearances, credentialing, and employment eligibility requirements
Preferred / Depending on Assignment:
- Home Health or community-based healthcare experience
- Experience supporting individuals with IDD, behavioral health, hospice, or chronic medical conditions
- Additional clinical certifications relevant to assigned programs or services
Education:
Graduate of an accredited or approved Vocational Nursing program required.
Experience:
- Minimum of one (1) year of nursing experience preferred
- Experience in home health, community health, hospice, long-term care, clinic, behavioral health, IDD services, or related settings preferred
- Familiarity with clinical documentation systems, medication administration, infection control, and person-centered care practices preferred
Technical Skills:
- Proficiency in Electronic Health Record (EHR) systems and clinical documentation platforms
- Ability to complete and maintain accurate healthcare documentation
- Basic computer proficiency including email, spreadsheets, mobile applications, and healthcare software systems
- Ability to utilize electronic scheduling, communication, and documentation systems
Languages:
English proficiency (spoken and written) is required. Bilingual or multilingual abilities may be preferred depending on client and program needs.
Driver License:
Valid driver’s license, reliable transportation, current automobile insurance, and acceptable driving record are required for field-based assignments. Employees must be able to travel between assigned client homes and service locations.
Travel Requirement:
This is a field-based in-home healthcare position requiring travel to clients’ homes and community locations throughout assigned service areas. Travel between client homes, healthcare facilities, pharmacies, provider offices, and other service locations may be required.
