Job Title: Home Health Aide
Job Overview:
We are seeking a compassionate and dedicated Home Health Aide to join our team. The ideal candidate will provide essential care and support to individuals in their homes, ensuring their safety, comfort, and well-being. Follow plan of care and communicate to IDG
Duties:
- Assist clients with activities of daily living, including bathing, dressing, and grooming
- Provide companionship and emotional support to clients
- Monitor and report changes in client's health status to the appropriate medical personnel
- Support clients with mobility issues and transfers
- Maintain accurate and detailed medical documentation
- Follow HA assignment.
- Communicate with IDG Team for every visit.
-Submit documentation in a timely manner.
Requirements:
- Experience in hospice care
- Knowledge of medical terminology
- Ability to lift and move clients safely
- Compassionate and patient demeanor
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
-Familiar with HospiceMD program
Per diem position.
Pay: $35.00 - $45.00 per hour
