REENTRY INTAKE COORDINATOR
Role Overview
The REENTRY INTAKE COORDINATOR (RIC) is the tactical engine of CWC, the ‘functional glue’ who manages processes, schedules and logistics during ‘Reentry Bootcamp’, at least for the first 30-60-90 critical days after release from prison.
The RIC is one of the first, if not the very first person that a returning citizen meets as they begin the ‘outside the walls’ portion of the reintegration journey. They are the primary navigator, managing the transition from incarceration into freedom.
The RIC’s purpose is to help stabilize each client by securing essential and immediate social services to aid in removing barriers to long-term success, ultimately preparing them for full-time, living-wage employment.
The heart and character of the RIC must reflect patience, kindness and empathy, but also exhibit strength, firmness and be the voice of reason in challenging situations.
Why This Role Matters
The first 90 days are the most unsettling time of the nine to eighteen month Startin’ Out Seamless Reentry Discipleship program. By providing a structured, supportive bridge, you aren’t just managing a case – you are helping individuals overcome anxiousness and worry about their futures, to settle in and reclaim their freedom.
Key Client Services Responsibilities
1. Intake & Assessment (Days 1-7)
- Confirm release time with TDCJ parole and coordinate client pickup from prison release unit.
- Complete all initial Intake applications/assessments/agreements, and initiate case management database profile for each new program resident.
- Conduct comprehensive screenings and conversations to identify immediate needs (identification, medical/mental health, food, clothing, etc.).
- Walk new arrivals through orientation using Crosswalk Covenant Handbook
- Help clients develop a personalized Reentry Success Plan with clear 30, 60, and 90-day milestones, leading to the 9-month Graduation Milestone.
2. Intensive Case Management (The First 30 Days)
- Progress Meeting: Weekly review of personal Reentry Success Plan with client to discuss any issues and progress status.
- Documentation: Assist clients with scheduling appointments or collecting information to obtain vital records (ID, Social Security cards, birth certificates).
- Resource Connection: Coordinate and schedule appointments, transportation, and logistics for immediate access to all needed social service agencies.
- Employment Ready: Prepare program participants to enter the workforce as part of their CrossWalk Center re-entry plan, ensuring all documentation required for employment is available; ID, Birth Certificate, education certificates, employment records and/or resume.
- Stabilization: Link clients to trauma-informed counseling, substance abuse support, or healthcare providers, whether through programmatic Volunteer Partners or Community Partners.
3. Career Readiness & Advocacy (Days 30-90)
- Skills Gap Analysis: Identify training needs required to secure employment in high-growth industries that offer living wages.
- Work Readiness: Aligning with a client's Reentry Success Plan, by coordinating attendance in training and or workshops with other social service organizations or Partners on professional communication, digital literacy, and how to explain the incarceration “gap" to employers.
- Placement Strategy: Work with Fair Chance CWC partner Employers to aid clients that are transitioning from stabilization into their first full-time role.
- Pre-employment: Assist client with banking; setting up accounts, online portals, ACH program fees set up and authorization, and repayment plans for arrears, prior to the first day of employment.
4. Tracking & Compliance
- Communicate with TDCJ Parole to set or resolve scheduling issues for clients.
- Maintain meticulous digital records of client progress and service delivery.
- Monitor client recidivism risk factors and provide proactive crisis intervention and report to the supervisor to intervene.
Other Coordinator Responsibilities
- Oversee House Leadership Team to ensure scheduled execution and reporting of:
o Drug screen at intake, and randomly, House Roster of Duties, House Inspection, House Meeting Agenda, House Maintenance Reporting, Intake House “Closet and Supplies” Inventory Process
- Engage residents in Volunteer opportunities
- Report on house maintenance, repair, and mechanical issues to CWC operations
- Enter and maintain all intake information in CWC databases; compile and regularly update all participant statistics
- Ensure intake homes within the Houston City limits are inspected, recertified and permitted annually
- Serve as a positive role model, exhibiting behaviors that support a respectful environment, free of drugs, and alcohol where all clients are treated equally with respect.
- Adhere to, teach and enforce the residential home program Standard Operating Procedures set for all clients, and to build and maintain a positive Community Culture.
- Meet and coordinate weekly with the broader Program Operations Team to ensure coverage for ALL client appointments and needs, by leveraging logistics and use of passenger van fleet and van driver schedules.

Education, Experience & Knowledge
· Degree in Business Administration, or three years' experience with weekly data tracking and analysis as leading indicators of performance preferred
· Degree in Human Services, Social Work, experience in the Recovery space strongly preferred
· Three years’ experience with case management in social work, counseling, recovery or reentry services (experience with justice-impacted individuals is a plus).
· Business degree or three years’ proven direct business experience is a plus
· Minimum three years' experience working with minority populations.
· English/Spanish is a plus
· Computer literacy is required: Microsoft Office 365: MS Word, Excel, SharePoint
· Data collection and recording experience required, coupled with math skills
Job Requirements
· Re-entry Service-based position – Houston, Texas
· Personal transportation is a must as travel is required for the job.
· Attendance at prison events (eligible to become TDCJ Trained Volunteer)
· Attendance at scheduled weeknight or weekend CWC events
