Family Choice of New York is seeking a Care Management RN to join our team of driven and compassionate team players. Our mission is to ensure members receive efficient, individualized care in the most appropriate setting, while supporting safe and effective transitions across the care continuum and helping ensure each member’s needs are met.
Key Functions:
The Care Management RN plays an essential role in coordinating high-quality, member-centered care in the most appropriate setting. This field-based position is responsible for completing in-home visits, maintaining regular communication with assigned members, and assessing ongoing care needs to ensure services are effective, timely, and aligned with each member’s individualized plan of care. Responsibilities include completing health risk assessments, developing and reviewing care plans, documenting visits and assessments accurately and timely, supporting safe transitions across the care continuum, and coordinating with members, families, providers, facilities, vendors, health plans, and community resources. The RN also identifies the need for social work intervention or additional medical services, ensures HIPAA requirements are maintained, completes required documentation in accordance with company policies, and supports the training and oversight of new team members. This role requires the ability to build and maintain collaborative, professional relationships with internal and external partners both by phone and in person.
Education:
- RN licensed in New York State
- Bachelor’s degree preferred
- Certified Case Management (CCM) preferred
Requirements:
- CPR certification
- Valid NYS Driver’s License and active automobile insurance to meet company requirements
- Transportation

Pay: $90,000.00 - $98,000.00 per year
