The Clinical Social Worker/LPC provides services to the organization in three broad areas, including services to the patient population, services to increase quality of healthcare planning and delivery, and services to the community to increase the efficient use of healthcare resources. Direct service is provided through the Rural Health Clinics. Direct service includes, but is not limited to: (1) interventions to increase psychosocial functioning; (2) adaptation to illness, injury disability; (3) post-hospitalization planning; (4) psychosocial assessment/evaluation; and (5) facilitation of services such as referral to community agencies, financial planning and provision of information and resource knowledge; (6) interpretation and communication of psychosocial needs to interdisciplinary team; and (7) works as needed with administrative direction to increase resources for patient/families.

Advise/Consult with family, physician, and other staff concerning needs of patient.
