Medical Coordinator - Developmental Disabilities (Group Homes)
Role Overview
Coordinate and manage all medical care services for residents with developmental disabilities in a group home setting. Ensure timely healthcare access, accurate documentation, medication compliance, and a safe, supportive living environment.
Key Responsibilities
Medical Coordination

Schedule and track all medical appointments, exams, and procedures

Attend appointments to obtain documentation and communicate care needs

Maintain accurate medical records and monthly appointment calendars

Coordinate with healthcare providers and emergency services
Medication Oversight
-
Manage medication ordering, storage, and supply levels

Monitor administration records (Therap) for accuracy and compliance

Conduct medication audits and support staff training on protocols

Provide basic medication education to residents
Health Plans & Compliance

Support development and update individualized heath plans

Ensure documentation meets DDD and regulatory requirements

Implement emergency medical response procedures
Resident Care Support

Assist with ADLs as needed (hygiene, meals, etc.)

Support dietary planning based on medical and personal needs

Promote a safe, structured, and nuturing home environment
Community & Coordination

Facilitate access to community activities and services

Transport residents to appointments and outings as needed

Encourage inclusion, independence, and quality of life
Qualifications

Experience working with individuals with developmental disabilities

Strong organizational and documentation skills

Valid NJ driver's license and clean driving record

High school diploma or equivalent
- Excellent communication and interpersonal skills
