What You'll Do
As our Clinical Liaison, you will:
- Build and maintain strong relationships with physicians, case managers, discharge planners, and referring hospitals.
- Assess patients for admission to our inpatient rehabilitation program using established clinical criteria.
- Educate patients, families, and referral partners about the benefits of inpatient rehabilitation.
- Coordinate smooth patient transitions from acute care to rehabilitation.
- Collaborate with physicians and interdisciplinary teams to ensure appropriate admissions.
- Represent Acadiana Rehabilitation Hospital in the healthcare community through outreach and networking.
Qualifications
Required:
- Current licensure as an RN, LPN, PT, OT, SLP, or other qualified healthcare professional.
- Minimum of two years of clinical experience in acute care, rehabilitation, case management, or a related healthcare setting.
- Excellent communication, relationship-building, and organizational skills.
- Ability to work independently while managing multiple priorities.
Preferred:
- Previous Clinical Liaison or rehabilitation admissions experience.
- Knowledge of inpatient rehabilitation admission criteria and payer guidelines.
- Experience working with physicians, hospital case managers, and referral sources.
