JOB DESCRIPTION
LICENSED PRACTICAL NURSE -SUPERVISORY VISTS
REPORTS TO: DIRECTOR OF NURSES (OR SUPERVISING RN IN THE
ABSCENE OF THE DON)
POSITION SUMMARY:
Provides nursing services in the patient’s home in accordance with Medi CareGivers, LLC standards and applicable laws and regulations. Ensures that patients receive the highest possible quality of care.
Any Licensed Practical Nurse must meet the following minimum qualifications established by the State of Georgia:
1. Never have been shown by credible evidence (e.g. a court or jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually assaulted, exploited, or deprived any person or to have subjected any person to serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application;
2. Participate in the orientation and training; and
3. Not have made any material false statements concerning qualifications requirements either to the Office of Regulatory Services, Department of Community Health (DCH) or
Medi CareGivers, LLC.
POSITION QUALIFICATIONS:
- Graduate of an approved school of nursing, currently licensed as a Licensed
- Practical Nurse in the State of Georgia
- Car with required proof of insurance coverage and a Georgia Driver’s license
- Social Security card or birth certificate
- Current CPR certification and PPD (tuberculosis inoculation)
- Knowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentation.
- Not been convicted of a felony or misdemeanor listed in the Rules and
Regulations for PHCP Chapter 290-5-54-.09(3)(a)1
- Sign an affidavit stating that you have never been found to have participated in the types of misconduct described by PHCP Chapter 290-5-54-.09(3)(a)1
- Pass background check and drug test

LICENSED PRACTICAL NURSE
PHYSICAL REQUIREMENTS:
- Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
- Must be able to stoop and bend to lift and transfer patients
- Lift up to 50-75 pounds.
- Able to work in a stressful environment.
- Able to assess and communicate with ill patients, co-workers, and general public.
- Is neat in appearance and practice, with good personal hygiene.
DUTIES AND RESPONSIBILITIES:
- Ensures quality and safe delivery of homecare services.
- Implements current nursing practice.
- Fulfills additional performance responsibilities as assigned and as necessary.
- Promotes effective written/verbal communication.
- Gives accurate information to patients and patient families regarding services.
- Provides information regarding equipment and supply needs.
- Uses equipment and supplies effectively and efficiently.
- Complies with Medi CareGivers, LLC policies and procedures.
- Promotes and maintains an agency environment that is in compliance with federal state, and local regulatory agencies
- Promotes education for homecare staff members.
- Promotes education for patients, patient families, and the community.
- Participates in personal professional growth and development.
- Completes all required documentation in accordance with the standards of Medi
CareGivers, LLC, and regulatory requirements.
- Works effectively with the medical staff, other members of the homecare team and administrative personnel. Maintains a cooperative working relationship.
- Performs other duties as assigned.
