Description:
We are looking for a compassionate case manager to join our Behavioral Health Home Case Management team. We seek a case manager who is willing to assist and support children, adults and families in Waldo and Knox Counties.
The right candidate will truly love helping people and will have strong time management skills. Clients will be seen in person and case managers make their own schedules and may work remotely once fully trained.
Pay is determined by case load and varies.
Responsibilities
- Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
- Handle case assignments, write treatment plans and progress notes, review case progress and determine case closure
- Help clients achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc.)
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations with team members throughout the entire case management process
- Meet with your clients regularly in their home and local communities
- Link clients and families with local resources, agencies and providers

Requirements:
- Bachelor's in human services or related field
- MHRT-C required
- Minimum of 1 year of case management experience
