Serve as the primary care provider for patients within assigned facilities, including B&C homes, ALFs, and SNFs.
Conduct comprehensive patient assessments, evaluations, and follow-up visits in a mobile/field-based environment.
Order, interpret, and evaluate diagnostic tests to identify and assess patients’ clinical conditions and healthcare needs.
Diagnose and manage acute and chronic conditions while developing appropriate treatment plans.
Prescribe medications and other therapeutic treatments in accordance with state regulations and clinical guidelines.
Develop, implement, and document individualized patient care plans.
Provide health education, disease prevention counseling, and lifestyle guidance to patients and caregivers.
Collaborate with physicians, nurses, and other healthcare professionals as part of an interdisciplinary care team to coordinate patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards.
Travel to assigned facilities within Los Angeles and Orange County, with current coverage needs in the San Fernando Valley and West Los Angeles regions.
