Title: Assistant Director of Nursing – Crescent Grove Personal Care / Grove Pointe Assisted Living
Reports To: Director of Nursing/ Executive Directors of Crescent Grove Memory and Personal Care and Grove Pointe Assisted Living
Status: Full Time, Non-Exempt
Work Hours: Normal business hours M – F, and other hours as needed by supervisor to meet the business needs.

Primary Purpose:
The Assistant Director of Nursing is a professional licensed nurse who has developed managerial skills through experience and/or education and who possesses nursing expertise in the clinical area. She/he is directly responsible to the Director of Nursing / Executive Director as she/he endeavors to maintain a safe and therapeutic environment for all residents. Medical records will be kept in accordance with facility procedures and Director will report resident needs/concerns to administration.
Qualifications/Skills Needed:

Licensed nurse with a Kentucky license in good standing is required.
- High School diploma or GED.

One year of long- term care (LTC) experience in a leadership role preferred. Medical records experience preferred.
- CPR Certification required.
- Experience creating collaboration with other departments and ability to organize, plan and direct nursing service is key.
- Must have patience, tact, cheerful disposition and enthusiasm to communicate effectively with residents in all levels of care, family members and employees.
- Must be able to assume the position of Director of Nursing during the Director’s absence.

Must be detail-oriented and self-motivated with ability to manage own time.
- Knowledge of medical terminology.

Proficient knowledge in Windows and Microsoft Office. Confident data entry skills.

Ability to problem solve, work on deadlines and follow written/oral instructions.

Maintain clean, orderly and safe working environment.

Must be at least 18 years of age.
Primary Duties (Essential Functions) include but are not limited to:
- Management of survey entrance sheet.
- Assumes delegated managerial responsibilities in the absence of the Director of Nursing.

Collaborate with Staff Coordinator with scheduling management for floor staff on all units.
- Supports the policies and procedures of the Nursing Service and Medical Records.
- Assists with planning, directing and supervising nursing care.

Collaborates with medical staff through informal and formal meetings.
-
Plans and participates in in-service training and orientation programs.
-
Makes routine rounds observing care practices. Responsible for resident safety.

Assumes responsibility for individual resident care when needed.
-
Plans, supervises and evaluates total resident needs, nursing care and progress by making daily rounds.

Provides assistance, direction and teaching of nursing care to all nursing personnel.

Establishes working relationships with Providers to develop coordinate and render medical treatment and nursing cares

Supervises and directs preparation and maintenance of residents’ clinical records.

Participates in Care Plan conferences in absence of Director of Nursing.
- Evaluates individual work performance through observation.
- Provides for and promotes good communication within and between shifts.

Reports equipment in need of repair, completing work orders as necessary.

Continuously evaluates the needs and services related to nursing service and resident care.

Collaborates with other departments and consults as needed.

Reviews concerns and grievances and reports to the Executive Director.

Files medical records promptly and accurately, scanning into electronic health record as appropriate.

Processes admission and discharge records timely and accurately.

Receives and files all diagnostic reports promptly and accurately.

Retrieves resident medical records from outside providers as needed.
- Audits transfer/discharge information for accuracy and completeness. Assists with data entry, printing and processing of computer forms, as assigned.

Releases medical records promptly upon request by authorized individuals in accordance with HIPAA guidelines.
- Audits medical records as assigned. Assists in development of Action Plans and/or Performance Improvement Plans for medical records audits not meeting compliance.

Assumes responsibility for compliance with federal, state and local regulations.
- As an LPN or RN, participates in the Nursing On-Call program.

Assists in covering the schedule when staff challenged.

Follows facility Residents’ Rights policies and procedures at all times.

Other duties as assigned.
- Provide Medical Records services to Grove Pointe Assisted Living.
