Position: Field RN PRN
Position Details:
- Role: Field RN PRN

- Pick up On-Call (Or don't. Not a requirement.)

Compensation
- Per Visit Rates (Varies on Visit Type)
- On-Call Rates:
◦ Week Day: $2 per hour (Plus hourly and/or visit rate if called)
▪ Monday 5:00 pm – Tuesday 8:00 am
▪ Tuesday 5:00 pm - Wednesday 8:00 am
▪ Wednesday 5:00 pm – Thursday 8:00 am
▪ Thursday 5:00 pm – Friday 8:00 am
◦ Weekend: $3 per hour (Plus hourly and/or visit rate if called)
▪ Friday 5:00 pm – Monday 8:00 am
◦ Holiday Rate: $5 per hour (Plus hourly and/or visit rate if called)

Qualification
1. Valid RN License: Current and unrestricted RN license in the state of practice
2. Experience: 2 years of RN experience, 1 year of home health experience (required),
3. OASIS Assessment: Working knowledge of the OASIS Assessment (required)
4. Clinical Skills: Proficient clinical skills and knowledge of home health practices, including assessments, wound care, medication management, and disease management.
5. Case Management Expertise: Strong understanding of case management principles, including care coordination, admission, discharge planning, and utilization review.
6. Documentation and Charting: Ability to accurately document patient assessments, interventions, and outcomes in compliance with agency policies and regulations.
7. Communication Skills: Excellent verbal and written communication skills to effectively collaborate with patients, families, healthcare providers, and interdisciplinary team members.
8. Critical Thinking: Strong analytical and critical thinking skills to assess patient needs, develop care plans, and make sound clinical judgments.
9. Empathy and Compassion: Demonstrated empathy, compassion, and patience in working with patients and their families during challenging times.
10. Time Management: Effective time management skills to prioritize tasks, manage caseloads, and meet deadlines in a fast-paced environment.
11. Interdisciplinary Collaboration: Ability to work collaboratively with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure holistic and coordinated care.
12. Adaptability: Flexibility to adapt to changing patient needs, schedules, and caseloads while maintaining high level of professionalism and quality of care.
13. Computer Skills: Proficiency in using electronic medical record (EMR) systems, documentation software, and other technology tools relevant to case management and home health practice.
14. Regulatory Compliance: Knowledge of relevant state and federal regulations, including Medicare guidelines, to ensure compliance with documentation, billing, and quality assurance standards.
15. Patient Advocacy: Strong advocacy skills to empower and support patients in making informed decisions about their care and accessing community resources.
16. Continuing Education: Commitment to ongoing professional development and staying updated on the latest advancements, best practices, and regulations in home health and case management.
