Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor patient health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals.
- Support patients with specialized care needs including Alzheimer’s disease, dementia, and memory care.
- Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a safe and comfortable environment.
- Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met with compassion.
Skills
- Proven experience working with individuals with disabilities, developmental delays, or in assisted living environments.
- Strong knowledge of medical terminology, home health care assistance procedures, and geriatric patient care practices.
- Ability to perform patient observation accurately and recognize signs requiring immediate attention.
- Familiarity with home health agency operations and home & community care standards.
- Maintain a CPR or BLS certificate.
- Excellent client communication skills; capable of providing compassionate personal care assistance while maintaining patient dignity.
- Ability to lift heavy objects safely when assisting with patient transfers or mobility; experience working with Hoyer lifts or similar equipment preferred.
