Essential Functions and Key Responsibilities:
Ø Oversee Day Habilitation Program services to ensure support is person-centered, respectful, and aligned with each person’s goals, preferences, and strengths.
Ø Promote choice, independence, self-determination, and meaningful community participation for people supported.
Ø Ensure daily programming supports skill development, social connections, employment readiness, and overall well-being.
Ø Develop, implement, and monitor individualized habilitation plans in accordance with OPWDD regulations and person-centered planning standards.
Ø Collaborate and clearly communicate with supported people, families, service coordinators, and community partners to ensure coordinated, responsive support.
Ø Hire, supervise, evaluate, and develop Direct Support Professionals (DSPs) to deliver high-quality, person-centered services.
Ø Ensure staffing patterns support individualized services and comply with OPWDD requirements.
Ø Provide direct support or program coverage when necessary.
Ø Ensure compliance with OPWDD regulations, HCBS Waiver requirements, and agency policies.
Ø Maintain oversight of health, safety, and emergency procedures.
Ø Ensure accurate, timely, and person-centered documentation of services and support.
Ø Develop and maintain community-based and volunteer opportunities aligned with people’s interests.
Ø Consistently maximize enrollment and daily attendance across day services.
Ø Participate in required meetings, training, and agency initiatives.
Ø Travel to programs, community events, and meetings as needed.
Ø Follow incident reporting procedure as per OPWDD to prevent abuse and neglect
Ø Perform other duties as assigned by Director of Community Services
Qualifications:
Ø Bachelor’s degree in human service field; Master’s degree in human services related field preferred
Ø Two years of Supervisory experience
Ø Experience working with the IDD population
Ø Experience working in a Day Program preferred
Physical Requirements:
Ø Strength and endurance for tasks like lifting, transferring, and assisting with personal care (bathing, dressing).
Ø Ability to bend, crouch, kneel, and stand for long periods.
Ø Occasionally lift up to 50 lbs.
Ø Stamina for CPR, physical interventions, and driving/transporting clients with mobility needs, all while maintaining a safe environment and responding to emergencies

Pay: $70,000.00 per year
