Job Responsibilities:
Our care management coordinators will coordinate and participate in activities related to care management services to family members and caregivers.

Job Qualifications:
- High school diploma or equivalent required.
- One to three (1-3) years of relevant experience required.
- Bachelor’s or master’s degree in relevant field strongly preferred.
- Health home or care management experience is strongly preferred.
- NYS driver’s license strongly preferred.
- Bilingual Spanish, preferred.
- Position is in Suffolk County.
The salary range for this position is $44,450.00-$69,340.00/year.*
