Job Summary
We are seeking a Licensed Master Social Worker (LMSW) to provide psychosocial support and care coordination for patients in an acute care setting. The LMSW will perform comprehensive psychosocial assessments, coordinate referrals, and assist patients with discharge planning and access to community resources, including Durable Medical Equipment (DME).
Responsibilities
- Conduct comprehensive psychosocial assessments.
- Develop appropriate discharge and care coordination plans.
- Refer patients to community resources and support services.
- Coordinate Durable Medical Equipment (DME) referrals as needed.
- Collaborate with physicians, nurses, case managers, and interdisciplinary teams.
- Maintain accurate and timely documentation in accordance with organizational standards.
- Advocate for patients and families to ensure continuity of care.
Requirements
- Master's Degree in Social Work (MSW).
- Active New York State Licensed Master Social Worker (LMSW) license.
- Minimum 1 year of LMSW experience.
- Experience performing psychosocial assessments.
- Experience with referral services and care coordination.
- Strong verbal and written English communication skills.

Pay: $60.00 per hour
