POSITION: Day Program QIDP
REPORTS TO: Director of Day Programming
RESPONSIBILITY: To maintain and improve the quality of life for adults with Developmental Disabilities in the Developmental Training Program by assisting in the management of the department.
CLASSIFICATION: Non-Exempt

QUALIFICATIONS:
٭ Bachelor’s Degree in human services or related field
* 1-year experience in the field of Developmental Disabilities
*QIDP experience and certification preferred
٭ Basic computer skills
٭ Satisfactory criminal background check
ESSENTIAL FUNCTIONS:
- Serve as QIDP for designated persons
- Maintain files
- Develop programming, including PCP’s, goals, ISPs, Behavior Plans etc.
- Coordinate or participate in staffings and IDT meetings
- Maintain case records for assigned caseloads per agency policy and state guidelines
- Work with team on admissions, transfers, and discharges
- Ensure a safe and secure programming environment
- Coordinate and communicate with outside agencies for support or services needed by persons in program
- Ensure programming is delivered in a manner consistent with the safety, dignity, and respect of those enrolled
- Maintain communications with guardians and family members
٭ Administration Duties:
- Maintain records and programming in compliance with DHS, Public Health, and other applicable guidelines
- Represent the department in the absence of DT Director
- Serve on various committees as needed
- Attend outside meetings and seminars for training and agency business
- Act as agency representative for client transitions from school program to adult services
- Maintain & keep record of personal CEUs
- Responsible for assistance in fundraising events

- Attend all internal meetings as required
٭ Direct Support:
- Assist with development of programmatic needs
- Implementation of program and goals of individuals as needed
- Assist consumers with mobility, personal hygiene, as needed,
(Ability to lift up to 50 pounds)
- Assist with consumer transportation
- Documentation as needed, filing of various reports including incident reports, consumer files, attendance, consumer tracking, etc.

٭ Training and certification:
- Complete QIDP training
- Must maintain “Q” certification with 12 credit hours annually
- CPR and First Aid certification
- Outside and in-house training appropriate for job duties
- Maintain valid driver’s license & Auto Insurance
- Complete annual training as directed

Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
